VitalMe - Patient care record management
VitalMe is Synapser's offline-first Personal Healthcare Record that syncs with the VitalMe cloud-based management platform, enabling a person to securely store and access their healthcare records on their mobile device, even when there is no internet connectivity. This provides patients with complete control over their health data and enables doctors to deliver responsive care.
With VitalMe, patients can easily create and manage their own personal healthcare record on their mobile device, while authorised doctors can access this information and provide tailored care through different modules. This includes the ability to issue prescriptions, track patient progress, create consultation notes, and refer patients to other healthcare professionals as needed.

Seamless Healthcare Record Management
As patients, we have all experienced the frustration of keeping track of our medical histories and records. It can be daunting to remember all the medications we’ve taken, diagnoses we’ve received, and procedures we’ve undergone. When we have to transfer our records from one healthcare provider to another, it can be a time-consuming and confusing process that could result in medical error. With no control over our own medical records, how can you ensure that the best possible steps are taken with regards to your health?
How Does It Work?
VitalMe is available for iOS or Android, and can be used on mobile or tablet, and allows users to authorise access to their preferred medical practitioner, who will then place the users record into a specific module based on their medical ailment. Each module has different protocols for the patient to follow, including scheduled appointments to attend and medication to take, while allowing the doctor to build a patient history.
VitalMe Integrates with Synapser’s proprietary Entry MFA, by encrypting the user data in Synapser’s Virtual Private Cloud, ensuring the protection of that data and medical records.
Key Modules:
VitalMe offers multiple configurations with the ability to load different modules for different ailments, with each module being designed to provide personalised health management based on the patient’s specific needs. These currently include, but are not limited to, Lynch Syndrome Management (Colorectal Cancer), Neurocognitive Functionality Assessment, and Orthopaedic Management.
User Benefits:
Patient:
Has access to and control of their personal healthcare record and can authorise who has access to it, including the ability to load and store a complete history of their consults, notes and medication that has been prescribed.
Can receive notifications from their general practitioner regarding health updates or actions that must be performed, such as following specific dietary requirements, taking medication, or performing certain exercises.
Can communicate via the app to the practitioner regarding their condition.
Doctor:
Can create and issue notes and prescriptions.
Can set up different testing and treatment templates for different illnesses and conditions, assigning to the patients accordingly.
Templates can be set up to send reminders to patients to perform certain actions.
Can review the patient’s engagement in the testing and treatment protocols and build a patient history.
Can refer the patient to other doctors.
If authorised by the patient, has full access to that patient’s complete healthcare history.
Receptionist:
Manages the doctor’s calendar by setting up appointments.
Can book patient consultations to run tests and enrol them into modules.
